The situation
A trust with a chronic flow problem and no capital budget. The official answer for years had been more capacity. Our brief was to find an answer that didn't need any.
What we did
We rebuilt the discharge pathway with the ward sisters who actually run it. We modelled the flow in a tool the trust's analysts could maintain. We sat in the morning huddle for three weeks running.
Outcome
The trust hit its winter targets without a single additional bed. The model is still in use, and the ward sisters present from it.
